Service Award Submission Form

The purpose of this award is to recognize service to the club by its members. The award is open to any member in good standing who accumulates 7 points by participating in the activities listed below. Please include documentation for non-club organized activities (i.e., WFA classes etc.). Only activities that took place after the adoption of this award at the 2008 annual dinner meeting count towards the award.

One event special staffed booth or event support	1/2 point
One year on the 3500 Club Board or Committee Membership	1/2 point
Two days staffing a Catskill Fire Tower via established club relationship	1/2 point
Event attendance at Wilderness First Aid Course	1 point
One year maintaining a trail section via established club relationship	1 point
One year maintaining a Catskill Lean-to via established club relationship	1 point
One Catskill Trail Crew participation via established club relationship	1 point
One 3500 Club Adopt a Trailhead event participation	1 point
One 3500 Club Adopt a Highway event participation	1 point
One 3500 Club Trail Maintenance event participation	1 point
